ID Policies + Arrival Protocols
ID Requirements:
Adults age 18+
Valid State issued Photo ID or Passport is required for everyone over the age of 18 years
Minors age 8+
Legal Guardian/Parent must be present with the following documents:
* Birth certificate or legally
binding guardianship document
* Guardian/Parent ID
-State issued ID or Passport
* Minor photo ID (one form):
-State issued ID, Passport, Childsafe ID card or School ID
A few things of note:
We can only accommodate one support person/additional guest to accompany you.
Have something to eat in the 4 hours prior to your appointment so you have healthy blood sugar levels.
